DALLAS TRINITY FC COMMUNITY TICKETS REQUEST

In consideration for the organization’s participation in Dallas Trinity FC’s Community Tickets Program, I, (insert your name), an authorized representative of the following organization (insert organization name), hold harmless Dallas Trinity FC, LLC, its officers and employees, from any liability which may arise in connection with participation in the Dallas Trinity FC Community Tickets Program, including but not limited to potential liability from accidents and injuries which may occur to participants attending a Dallas Trinity FC soccer match.

Furthermore, my organization agrees to indemnify Dallas Trinity FC, LLC, its officers and employees, from any suit, claim, or other action brought by any participant, participant’s parent(s) and/or guardian(s), and/or any other person related to the participant in connection with their attendance at or participation in any and all Dallas Trinity FC Community Tickets Program events.

This release shall apply to any such injury, accident, or liability, whether or not there is negligence on the part of Dallas Trinity FC, LLC, its officers or employees. This release further applies to any claim for contribution that the organization may have or assert against Dallas Trinity FC, LLC, its officers or employees, arising from any such injury, accident, or potential liability. I declare that the terms of this release have been completely read, are fully understood, and are voluntarily accepted.
